Observed Cole slaw in ice bath at front line at 54F and food items (cheese, Cole slaw, raw fish fillets) in top and bottom portion of prep top cooler not being held at or below 41F. Discussed with CFSM that ice bath must be maintained, and ice level must be around counter at or above food level. CFSM discarded Cole slaw with Cole slaw from reach-in cooler (temp 39F) and was checked at end of inspection (temp 41F). Cheese, Cole slaw, and fish from prep top cooler discarded and will not be used until maintenance can repair.

Discussed with CFSM and food (rice, green beans, mac n cheese) discarded. Steam well up front was turned on and water temperature increased from 125 to 149 during inspection and maintenance was called to come look at cabinet warmer (unit was ready low and was turned off and back on due to breaker tripping and status changed to preheat).
