Observed significant black build-up on walls of ware washing room. Observed multiple broken tiles with accumulation of food and debris behind loading area. Corrective Action: Discussed importance of keeping the warewashing room maintained and clean with PIC. All walls must be designed and constructed so they are smooth and easily cleanable. PIC will contact facilities to have broken tiles repaired and/or replaced. PIC will also designate employee to properly clean walls in warewashing room. The Health Department will recheck this in one month.

Observed potentially hazardous TCS food (grilled chicken wraps) cold-holding at temperature greater than 41 F in front food service grab-n-go coolers. Corrective Action: PIC notified and educated on proper cold holding and cooling procedures. Because these were prepared less than four hours ago, all wraps were moved to the walk-in freezer, lids removed and cooled to less than 41 F.

Corrective Action: PIC was notified and chose to discard the cheese. All other items in reach-in cooler were at or below 41 deg F. After further investigation, it was determined the cheese was likely left out on the counter top when they were preparing their cold-holding items for deluxe sandwich prep. Food service employees to be re-trained.

Corrective Action: PIC was notified. Their thawing cooler is currently out of service and the employees have been using the large walk-in cooler to thaw. As a result, the food service employees have been pulling large quantities of chicken out of the walk-in cooler and allowing them to sit at room temperature before being prepped, seasoned and placed in breading cooler. Because the chicken was found to have been out of temperature for less than 4 hours, both the original and spicy chicken breasts were brought to the walk-in freezer to cool back down to less than 41 deg F. All food service employees will be educated and re-trained.
