Observed a black mold-like debris on the interior chute on the ice machine in the lobby. The unit must be cleaned before customers can start using it. It must be cleaned to sight and touch. 511-6-1.05(6)(n) - Manual and Mechanical Warewashing Equipment, Chemical Sanitization-Temperature, pH, Concentration, Hardness (P,Pf) (n) Manual and Mechanical Warewashing Equipment, Chemical Sanitization - Temperature, pH, Concentration, and Hardness. A chemical sanitizer used in a sanitizing solution for a manual or mechanical operation at contact times specified under subsection (8)(b)3 of this Rule shall meet the requirements specified in DPH Rule 511-6-1-.07(6)(g), shall be used in accordance with the Environmental Protection Agency (EPA)-registered label use instructions P, and shall be used as follows: 1. There was an underage child observed in the kitchen once the person in charge returned to the establishment. The initial person in charge advised that she could not provide any paperwork and she was no longer in charger. She advised that I have to speak with the CFSM that arrived 30 minutes after the inspection began. CFSM was advised that unauthorized personnel was not allowed in the food preparation or storage areas. Initial person in charge also advised that she changed the label after preparing a new batch of rice. The label was date 2/11/2026. There was also open sauces that were labeled 09/01/25. PIC advised that the labeled were stuck on the pan and could not be removed. PIC advised to use a new pan with the proper label. 511-6-1.03(2)(a)-(n)(p),(q) - Responsibility of PIC (Pf) (a)Operations Not Conducted in Private Home.
